End of Life Care

One of the forty-four recommendations of the Liverpool Care Pathway(LCP) Review (2013),  was that the LCP, known  locally to some people as the Integrated Care Pathway (ICP) for the Dying, should be phased out and replaced by an Individualised End of Life Care Plan by July 2014.

In response to this recommendation, a group representing members of the public and local professionals from a variety of clinical backgrounds and care settings, have reviewed national guidance provided by NHS England and the Leadership Alliance for the Care of Dying People, to inform the development of a ‘Care Plan for End of Life’ that will subsequently replace the LCP/ICP within local care settings from July onwards.

We would like to invite you to have your say on the draft ‘Care Plan for End of Life’ by completing this online End of Life Care Plan Consultation Survey  that has been designed to facilitate constructive feedback from a wide group of individuals.
